FORMA™: A Clinical Guide by Dr. Jorge Eustáquio

FORMA composite features a zirconia nano-hybrid formula that allows for an exceptionally durable restoration with better initial and retained gloss. The silanated zirconia particles in FORMA composite are rounded, giving a beautiful high gloss finish and enhanced durability to the restoration—making FORMA composite ideal for both anterior and posterior applications.

It is available in a full range of shades with ideal translucency, opacity, and fluorescence when compared with the natural characteristics of the tooth. FORMA composite’s excellent optical properties enable clinicians to reach the ideal balance between esthetics of direct restorations and dental substrates with the use of only one body color (OneStepColor), or through a combination of dentin, enamel, and effect shades for more specific procedures. A zirconia nano-hybrid formula gives FORMA composite excellent mechanical properties, high strength, and excellent working consistency. It is easy to sculpt and simple to polish, offering beautiful final esthetics.

Professor Jorge Eustáquio holds a master’s degree in Dentistry (São Leopoldo Mandic – SP) acting as coordinator of the Specialization Course in Dentistry at ABO – AL. In addition to his clinical experience, he continues to publish articles in national and international journals, authoring chapters in several books in Restorative Dentistry.

 

1. Female patient, 32 yo, seeking to replace resin composite veneers to obtain a more harmonious and whiter smile.

 

2. Whitening technique: association of Opalescence® Boost and supervised tooth whitening with Opalescence Go 10%, for 10 days.

 

3. After 15 days, re-anatomization with Forma™ composite system, seeking minimal intervention.

 

4. Removal of inappropriate restorations. Silicon index to guide preps, based on a wax up.

 

5. Palatal shell using Forma™ WE. Palatal silicon index used as reference.

 

 

6. Palatal shell view, with Forma™ A1D.

 

7. Forma™  A1B do replicate dentin (middle and incisal thirds).

 

8. Application of ocher and white pigments. Forma™ Trans for incisal translucent effect.

 

9. Final layer with Forma™ WE. All layers were properly light cured, following instructions for use.

 

10.  Finishing and polishing using sanding discs and Jiffy™ systemystem

 

11.  Final aspect. Immediate result. Textured and shining surface.

 

12.  5 years follow-up. Recall once a year for polishing.
